How to prepare
  • Fasting required 8–12 h before the test (water only) recommended for glucose, cholesterol/triglyceride accuracy
  • Sample timing: between 7:00 AM – 10:00 AM
  • Stay hydrated; avoid strenuous exercise, heavy meals, and supplements (iron, B12, multivitamins, biotin) for 48 hours prior
  • Continue prescribed medications unless otherwise directed

Who is this test for?

It is for adults who suspect low iron, whether from fatigue, breathlessness, hair shedding, restless legs or heavy periods. It measures ferritin, serum iron, transferrin saturation and total iron binding capacity alongside a full blood count.

Can I be iron deficient without being anaemic?

Yes, and it is common. Iron stores deplete before haemoglobin falls, so you can have significant symptoms while a standard full blood count looks entirely normal. Ferritin is the marker that identifies this earlier stage.

Does inflammation affect ferritin readings?

It does, and this is an important limitation. Ferritin rises during infection, inflammation and after injury, which can make it appear normal even when iron stores are low. That is why transferrin saturation and inflammatory markers are interpreted alongside it.

How should I prepare?

Test in the morning, avoid taking iron supplements for one to two weeks beforehand if possible, and postpone testing if you currently have an infection, as this distorts ferritin.

Why does low iron need investigating rather than just treating?

Because the cause matters. In men and postmenopausal women particularly, iron deficiency can indicate blood loss from the gut that requires investigation. Correcting the level without establishing the reason risks missing something significant, and your report will advise accordingly.
